In my ideal world, we would all be adept at establishing clear and compassionate boundaries, sharing them with others to nurture self-care and maintain empathy. Unfortunately, circumstances often arise where it might not be safe to openly communicate these boundaries, especially regarding political safety in work environments. This is prevalent in professions like medicine, where rigid hierarchies and toxic cultures persist.

Even when certain circumstances make quitting a certain job hard, those who find they’re in such situations must remember that deciding to go to work each day is a decision. Decide on what is and is not suitable for you. For instance, if the workplace expects physicians to accept walk-in patients, you can decide if this aligns with your priorities and life responsibilities. You don’t always have to announce your boundaries; instead, address situations as they arise without the need for extensive justification.

Normalize the idea that setting boundaries may surprise or disappoint others accustomed to your compliance. People may have various reactions, and it’s okay to let them have their responses without taking ownership of their displeasure.

By learning to set boundaries and say no, you exchange the burden of resentment and frustration for the discomfort of potentially upsetting others. Understand that this adjustment is part of the process.

If you’re uncomfortable vocalizing your boundaries to superiors or colleagues, it doesn’t mean you can’t establish them for yourself. It could be as simple as saying no to walk-ins, prioritizing short breaks during the day, or not substituting patient emails for appointments. Practice and persist, even when it feels challenging. It’s time to stop constantly giving away our time and energy and instead focus on self-care to maintain our well-being and compassion in our professional lives.

Keep in mind that having limits is necessary, not selfish.
